Título:Cervical branch of the facial nerve in leprosy.

Resumo:This study demonstrates that the platysma is occasionally palsied in leprosy and that this only occurs when the facial nerve already has some other palsy. That there needs to be a facial palsy before there can be a platysma palsy is strongly suggested, in that there was no case of an isolated platysma palsy. Patients, regardless of age or other factors, could mimic a platysma contraction. This obviates the need for electrical testing to examine for a platysma palsy. It also means that a nonfunctioning platysma on clinical examination is, in fact, a palsied platysma. While lagophthalmos is regularly examined for, and any obvious facial paresis would be noticed, less severe forms of facial muscle paresis will only be found if formally examined for. The mechanism whereby the facial nerve is involved in leprosy is not clarified, but our findings suggest that proximal spread of a lesion that began in the zygomatico-temporal branches and reaches to the facial nerve trunk is more likely than new lesions developing de novo in other peripheral facial nerve branches. That the primary lesion is within the facial nerve trunk in all cases but we only see the frequent zygomatic sequelae due to secondary factors is not excluded. (AU)^ien.

Resumo:A case of facial palsy with lagophthalmos with exposure keratitis was corrected surgically by a method of temporalis transfer. When the slings were tightened leaving 1 cm gap in the palpebral aperture, lagophthalmos persisted. A secondary tightening procedure causing overlap of the upper lid over the lower yielded good results.

Título:Results of surgical correction of lagophthalmos (Gillies technique) in leprosy patients.

Resumo:The results of the temporal muscle tranfer (Gilles technique) in 51 eyes (34 patients) have been analysed. The main objective was to evaluate the degree and time needed to recover voluntary occlusion and the static effect of transfer on the lower eyelid in cases of partial ectropium and epiphora. In 34 (66,67 por cento) eyes there was completed and lasting voluntary eylid occlusion. The medians of time to obtain complete eyelid closure while were 8 days (1-120) in the excellent group and 14 days (1-120) in the good group. The correction of ectropium and epiphora was seen 15 (83,33 por cento) eyes. The recovery of voluntary eyelid closure and repositioning of lower eylid observed in the majority of our cases confim the efectiveness of the Gallies technique for correction of lagophthalmos. (AU).

Resumo:Leprosy is a mycobacterial nerve and skin infection, which can be eradicated by antibiotics. Some patients affected by leprosy, once cured, have residual nerve impairment with paralysis and sensory neuropathy. A series of patients with facial nerve paralysis, investigated using clinical, histological and electrophysiological techniques, demonstrated that the nerve pathology was distal to the section of main trunk prior to its bifurcation. Facial reanimation was achieved with a free gracilis-muscle transfer, coapting its motor nerve to the ipsilateral facial nerve trunk proximal to the site of the leprosy pathology, with a moderate clinical result. Resumo:CASE REPORT: A case of bilateral facial palsy with paralytic ectropion, lagophthalmos and corneal damage secondary to corneal exposure in a long-standing patient with lepromatous leprosy is presented. Correction of paralytic ectropion was performed by medial cantoplasty, tarsal strip and Medpor lower eyelid spacer implantation. Lagophthalmos was corrected by gold weight implant in the upper tarsus. DISCUSSION: Ocular findings in leprosy appear in 72 per cent of patients. Facial nerve palsy occurs in 3-19.8 per cent, being bilateral in 5 per cent. In long standing cases with corneal complications secondary to exposure, surgical treatment is required. (AU).

Resumo:Investigation of the eyes of 388 Turkish patients suffering from leprosy revealed some rare and interesting ocular lesions, including Melkersson-Rosenthal syndrome, acute diffuse plastic iridocyclitis, military leproma of the iris, an unique type of iris strophy that produces iris holes and hypopigment patches in the fundus and choroiditis leprosa´praecox. Histopathologic studies of the orbicularis oculi muscle in patients with lagophthalmos suggested that myositis rather than facial nerve paralysis was responsible for the clinical picture. Tonometric examination of 38 eyes showed abnormally low pressures, and tonographic studies of 49 eyes demonstrated that both aqueous production in leprosy patients.(AU).

As a consequence, surgical treatment is necessary and directed: --against lagophtalmos, with static or dynamic methods, --against the ptosis of commissura labiorum with several techniques available.(AU).

Resumo:Fifty-one Montangards in South Vietnam with proven leprosy were studied ophthalmologically. Of these, 39 had ocular involvement. The cornea was involved in 31 of the 39 patients. About the same percentage of ocular complications were seen in cases of lepromatous leprosy as tuberculoid and the mixel type. Five patients were blind.(AU).

Resumo:The incidence of facial deformity is high in leprosy. Once deformity has become established and is not reversible by medical therapy, it is correctable only by surgery. Plastic surgical procedures have been developed and are standardized for the correction of the common facial deformities due to leprosy.It is essential to include methods of plastic surgical reconstruction in a program of rehabilitation designed to assist these patients for their return to society. The plastic surgical correction of facial deformities in leprosy is most rewarding; although it is of lesser early importance than medical management, it is essential in the later rehabilitation of patients who have had leprosy.(AU).

Resumo:Ophtalmic findings suggestive of leprosy include loss of eyebrows and eyelashes, beaded corneal nerves, puntacte subepithelial superior limbal corneal opacities, entropion of the upper eyelids, conjunctival hyperemia an the other canthal area, iris pearls and unexplained unilateral or bilateral 7th nerve histories were presented. The average time interval between the initial presenting complaint the establishment of the correct diagnosis was six years. There is no accurate method of determining the real incidence of leprosy in the United States, but it would appear to be higher than suspected. Ophthalmologists should consider leprosy when examining patients with any of the findings just mentioned, particularly if the patient has a sssociated skin or neurologic findings or nasal stuffiness.(AU).

Resumo:Complete and partial nerve paralysis can result from leprosy. The latter is more prevalent and results in lagophthalmos and corneal hypoesthesia. The former is characterized by loss of facial expression, deviation of the face to the nonparalyzed side, difficulty in chewing, and drooling, in addition to lagophthalmos and corneal hypoesthesia. Affected patients are at risk to develop blindness, as well as suffering social and economic deprivation because of the effects of facial nerve paralysis. Prevention of blindness and amelioration of the latter were successfully achieved by muscle transfer procedures, temporalis transfer to the eyelid, and masseter transfer to the mouth and nasolabial fold in small, minimally equipped hospitals in Africa.(AU).

Resumo:The communication is based on the experience with 380 reconstructive operations in the face fo patients at the residual stage of leprosy. A classification of the sequelae of leprosy in the face, which includes all types of disfigurement and facilitates selection of the most rational method of surgical treatment for each group, has been elaborated. in most patients who have overcome leprosy, a combination of all kinds of deformations and defects in the facecan be observed, which requires a whole complex of plastic operations in order to take off the mask of leprosy. Reconstructive operations in the face are indicated at the stage of deep regression or in residual leprosy. The paper deals with the successful repair of deformations of the nose by means of dividing the subcutaneous scar contractures and of constructing a supporting cartilage scaffolding, of reconstructing the nose and lips with a Filatov pedicle flap, of repairing facila-muscle palsy with a tape of fascia and lagophthalmos with a myofascial-band plasty, of reconstructing the eye browns with arterialized flaps on subcutaneous pedicles, and of other reconstructive and cosmetic operations. The skin wounds have healed well in leprosy patients, and no hypertrophic or kelloid scar formation has been observed.(AU).
